I acknowledge and agree that:

1. I am between the ages of 12-18 and a registered member of Volleyball Ballarat or Volleyball Victoria
2. I acknowledge that Volleyball Ballarat is a volunteer organisation and I will be respectful and courtious of the event organisers and coaches. Where any behaviour is deemed to contradict this expectation I understand I can be asked to leave the camp and no refund will be provided. 
3. I consent to Volleyball Ballarat or event organisers acting on behalf of Volleyball Ballarat to coordinate any emergency medical attention should it be deemed necessary and contact is unable to be made with the emergency contact person provided during registration
4. I will be bound by and must comply with all Volleyball Ballarat and Volleyball Victoria Policies, including but not limited to, the Volleyball Australia National Integrity Framework and associated integrity policies, Anti-Doping Policy, Code of Conduct and Social Media Policy which can be found here – https://volleyball.org.au/integrity/policies/
5. I acknowledge that I am bound by these policies and it is my responsibility to be familiar with these policies;
6. A breach of these policies could result in further action being taken, including but not limited to, being asked to leave the camp or disciplined as outlined in the complaints or discipline procedures for the respective policy
7. I acknowledge that Volleyball Ballarat is also bound by Volleyball Victoria policies and has taken all appropriate steps to ensure this is a childsafe event, but if I have any concerns I can contact president@volleyballballarat.com.au to discuss these.
