Rep Player Registrations are to be completed by all players selected for a Toowoomba Hockey representative squad or team. 

Rep Player Registrations include:

• Confirmation of Selection - this may include a Confirmation of Selection fee - this fee will be deducted from the players total rep fees. 
• Player Contract
• Emergency Information
