If you have been involved in an incident or have observed an incident, please complete the attached form as soon as possible or complete on-line here

When complete, please inform the Club's Safety Officer, or Captain at captain@commercialrowing.com and provide them with a copy of the completed form.

The Safety Officer or Captain will inform Rowing Qld if thr on-line form has not been completed.
