Paraburdoo Amateur Swimming Club

1. I agree to abide by the rules, regulations and policies of Paraburdoo Amateur Swimming Club inc, Swimming WA, Swimming Australia, the relevant Regional Swimming Association and the relevant club, including Swimming Australias Anti-Doping, Member Protection and Privacy Policies (these are available at www.wa.swimming.org.au)

2. PARENT PERMISSION FOR USE OF PHOTOGRAPHS & VIDEO RECORDING : I do hereby grant permission to Paraburdoo Amateur Swimming Club to use the image of my child. Such use includes the display, distribution, club events, training sessions, swim meets, and any occassion involving Paraburdoo Amateur Swimming Club, such as publication, transmission, presentations or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures, handbooks and newsletters, videos, and digital images such as those on the Paraburdoo Amateur Swimming Club Inc web site or Facebook page.

If for any reason, you do not wish to have your childs photograph and/or video used for any of the abovementioned purposes, notice must be provided in writing to the club at email: Paraburdoo swim club
