Terms Of Registration

Thank you for nominating for a position in a NDWHA Representative Team. By nominating and electronically submitting this form, you and your parent/guardian agree to the following conditions:

1. I understand the dates of the State Championships and confirm that I will be available for all scheduled days of this event.

2. I understand that there are costs associated with nominating for a NDWHA Representative team. To trial for selection incurs a fee of $10, and, if selected, the registration cost is $185. I understand that this fee needs to be paid at least two weeks prior to the State Championships or my position in the team may be declared vacant. Additional training sessions will be $10-$15 each. Accommodation for the State Championships needs to be sourced by the players. If additional carnivals are scheduled, this will incur an additional cost (approximately $20/player).

3. If I am successfully selected in a Representative team and subsequently decide to withdraw from the team for any reason prior to the commencement of the State Championship, then I understand that I will forfeit the representative levy. Exceptions will be made for medical reasons, providing a certificate is supplied.

4. The Team Coach has the final decision on the starting line-up for all games, player positions and structure and all player interchanges. Although all effort will be given to ensure players receive equal game time, I understand that depending on the game strategy employed by the Coach, this may not occur.

6. I, and my parent/guardian if under 18yrs, agree to abide by all requirements and requests made by the Coach, Manager or an NDWHA official at all times. We have read the NDWHA Code of Conduct, below and https://www.ndwha.com.au/policies, and agree to abide by this Code.

7. In the case of a medical emergency, my parent/guardian provides authority for the Coach / Manager or other NDWHA Official to provide any assistance deemed necessary – including a request for an Ambulance

 

NDWHA Code of Conduct Policy

This Code of Conduct (the Code) should be read in conjunction with the Hockey NSW Code of Conduct provisions, the NDWHA Rules of Competition and the Constitution of the NDWHA.

This Code of Conduct applies to all players, members, parents, spectators, officials and office bearers of affiliated clubs and organisations participating in, or in attendance at, competitions or matches conducted under the control of, or when representing, the Newcastle District Women's’ Hockey Association.

Persons subject to this Code shall not:

  1. Assault or attempt to assault, lay a hand upon, shove or strike an Umpire, an Official, another player or spectator.
  2. Abuse, either verbally or physically, in person or online, an Umpire, an Official, another player or spectator.
  3. Dispute, as distinct from question, decisions made by an umpire, selector, official or a member of the NDWHA Board.
  4. Use crude and/or abusive language, or gestures, or engage in any form of conduct detrimental to the spirit of the game, or likely to bring the game into disrepute.
  5. Appear on the field of play at any time intoxicated or under the influence of drugs or be in the possession of alcohol or drugs on the field of play.
  6. Refuse to supply full name and registered address if required by an Umpire or Official for any reason.
  7. Comment publicly, either verbally or in writing, with a member of the media or with a person knowing that it may be reported in the media, including posting on social media, on any matter considered likely to bring the game into disrepute.
  8. Engage in any other conduct, either on, or in the vicinity of, the field of play, considered prejudicial to the best interests of hockey or in contravention of the Constitution and Regulations of this Association.
  9. Act in a manner contravening the Hockey Australia National Safe Hockey Policy and Procedure, Play by the Rules and NDWHA Inc. Use of Electronic Media and Social Networking Sites Policy.

Should a person, as categorised above, breach the Code of Conduct, the process outlined in the NDWHA Rules of Competition will be followed.
