I, [Parent/Guardian Name], hereby consent to allow my child, [Child's Name], to participate in the Holiday Spring Sport Spectacular organized by Newtown City Hockey Club. I understand and acknowledge the following terms and conditions:
Assumption of Risk: I acknowledge that participation in the holiday program involves inherent risks, including but not limited to physical injury, accidents, and exposure to contagious illnesses.
Release of Liability: I waive and release any and all claims, demands, and liabilities against Newtown City Hockey Club, its employees, volunteers, and affiliates for any injuries or damages arising from my child's participation in the holiday program.
Emergency Medical Treatment: In the event of an emergency, I authorize Newtown City Hockey Club staff to seek medical treatment for my child. I also agree to bear all expenses associated with such treatment.
Supervision: I understand that Newtown City Hockey Club will provide supervision during the holiday program activities. However, I acknowledge that it is not possible to guarantee absolute safety.
Media Release: I grant permission for my child's likeness (photographs, videos, etc.) to be used by Newtown City Hockey Club for promotional purposes related to the holiday program.
Personal Belongings: I understand that Newtown City Hockey Club is not responsible for any loss or damage to personal belongings brought by my child to the holiday program.
Compliance with Rules: I agree that my child will adhere to all rules and instructions provided by Newtown City Hockey Club staff during the holiday program.
