The Under 18 Boys State Indoor Championships will be held in Brisbane from 12-14 December 2020 at Brisbane Indoor Hockey venue (Kedron SHS Indoor Sports Centre)

If you are unable to nominate online, please contact MDHA (mdha@bigpond.com)

• Nominations after the closing date will only be accepted at the discretion of the Maryborough Indoor Hockey Committee

• Participation fee will be payable upon selection

• You will receive an email confirming your nomination has been successful
• Complete Medical History Form, return to Manager upon selection (download available below)
