The Under 16 Girls State Indoor Championships will be held in Maryborough from 25-27 October 2024.

• Nominations after the closing date will only be accepted at the discretion of the Maryborough Indoor Hockey Committee

• Payment of the nomination fee of $50 is required and will be refunded if not selected

• You will receive an email confirming your nomination

• Complete Medical History Form, return to Manager upon selection (download available below)

• Abide by MDHA Code of Conduct at all times (download available below)
