The Qld Super Six Open / Under 21 Men Indoor Championships will be held in Brisbane from 12-14 November 2021 at the Brisbane Indoor Hockey Venue at Kedron.

• Nominations after the closing date will only be accepted at the discretion of the Maryborough Indoor Hockey Committee

• Participation fee will be payable upon selection

• You will receive an email confirming your nomination
• Complete Medical History Form, return to Manager upon selection (download available below)
• Abide by MDHA Code of Conduct at all times (download available below)
