The Under 15 Girls State Indoor Championships will be held in Maryborough from 29-31 October 2021 at the Maryborough Indoor Hockey Complex.

• Nominations after the closing date will only be accepted at the discretion of the Maryborough Indoor Hockey Committee

• Participation fee will be payable upon selection

• You will receive an email confirming your nomination
• Complete Medical History Form, return to Manager upon selection (download available below)
