All playing or training members of Wolves Water Polo Club are required to complete the Wolves Water polo Code of Conduct and Behaviour Form, the Wolves Water polo Medical Consent form and Photography Release Form at time of registration.

All Medical Consent Forms are re-assessed prior to each competition. Players are required to advise the club of any new relevant information as it presents itself.
