This form is designed so we have an up to date record of details (including bank account to pay your per diem) and any medical conditions or allergies you may have. For those travelling from out of Christchurch your flight details will mean we can arrange airport pick ups etc. 

Following Fencing Central's lead in an effort to combat climate change, Mid South will no longer be providing disposable water bottles or disposable coffee cups for the officials and instead encourage everyone to bring their own.
