I warrant that, to my knowledge, I am medically and physically fit and able to undertake and participate in water polo activities and will not be a danger to myself or to the health and wellbeing of others. I understand that while participating in Central Newcastle Water Polo activities I must ensure I comply with social distancing and follow directions given to me by Central officials and/or coaches.
