This form must be completed by any player seeking a Short Term Clearance from Brisbane Hockey Association to play for another Association at a State Championships. Questions must be answered truthfully.

All requests will be reviewed by the BHA Board. Completion of this form does not guarantee that a Short Term Clearance will be provided.

For each application submitted to and approved by BHA, the details are manually transfered to a restricted access Hockey Queensland (HQ) form to be processed and approved/not approved by them. A list of HQ approved Player Clearances will appear on the HQ Website.
