No more lost items on those cold winter days- you can have your name embroidered under the club logo for $1 extra and you can also have AHC printed on the back for an extra $8.

You can order by EITHER

1/emailing the club at alstonvillehockeyclub@hotmail.com with "Merchandise" in the subject line and in the body of the email list the item(s) required, the size and whether the extra "name" and "AHC" options are requested. Please state the actual "name" that is to be embroidered under the club logo if applicable

OR

2/ printing the attached order form, filling it out and giving to a member of the executive (Mark Harris, Debby McClelland, Ann Simon or Sheryle Hounsell) by Friday 10th May.

ALL PAYMENTS are to be made by direct transfer to the clubs account before Monday 13th May 2019.  

Once orders are received you will receive an invoice from the club with the bank payment details.
